Women, Infants and Children (WIC)

The Women, Infant and Children (WIC) Nutrition Program is a Federal program that provides food and nutrition information to pregnant, postpartum, breastfeeding women, infants and children under the age of 5.

Participants Receive

  • Breastfeeding support
  • Nutrition education and information
  • Nutritious foods
  • Referrals to other Public health, community, and health services as needed


  • Participants must be residents of the state of Wisconsin.
  • Pregnant women or new mothers, infants up to age 1, or children up to the age of 5 who are income eligible. (Eligibility guidelines are set by the government and change each year)

Where do I go?

WIC is administered to our community through a partnership with the Cudahy Health Department. The Greenfield Health Department serves as a satellite clinic.

WIC clinics are held 6 times a month at our facility.
WIC Appointments:
  • 8:30 a.m. - 2:45 p.m.
  • 1st, 2nd, 3rd, & 4th Mondays and 1st, 2nd, & 4th Tuesdays
Check Pick Up:
  • 8:30 a.m. - 3:30 p.m. (on same dates as WIC appointments)

What do I bring along to my first appointment?

  • Current proof of income
    • Paycheck stub for all working in the household or a letter from your employer
    • Tax return if self employed
    • Unemployment form or copy of check
    • Notice of Decision letter for food stamps, medical assistance or W2
    • Forward Card (Title 19 card for medical assistance, Healthy Start or Badger Care)
  • Identification for the persons being certified and for the guardian
    • Drivers license
    • Birth certificate
    • Old WIC folder
    • Infant crib card, bracelet or hospital certificate (not Social Security cards)
  • Immunization records if available
  • Proof of Pregnancy
  • The completed WIC questionnaires
  • The person being certified

At the appointment there will be:

  • Blood testing
  • Food vouchers issued
  • Forms to fill out
  • Nutrition assessment and counseling
  • Weight and height measurement
